Font: Pf Tempesta Five condensed, 6pt and anti-aliased everywhere
Theme: Windows classic + 3dcc (jasmin 3d color changer, you'll get a link here on dd)
The navigation bar (back and forward buttons,navigation window and search window buttons) are hacked browseui.dll. There are some very interesting tuto for vista and seven here on da. As for xp you can use styler toolbar or DES hacks

I am not using a typical icon but rather an icon patch, it's Tango Patcher 6000 8.03 by ~vertigosity on deviantART for vista from ~vertigosity
There are other versions for 7 or xp
